AUTHORIZATION FOR RELEASE OF INFORMATION

I hereby authorize all persons or companies in the categories listed below to release, without liability, information regarding employment, income and/or assets to the Owner, Landlord or Agent identified below for purposes of verifying information on my rental application.
INFORMATION COVERED: I understand that, depending on program policies and requirements, previous or current information may be needed. Verifications and inquires may include, but not be limited to:
  • • Credit and Criminal Activity
  • • Resident and Rental Activity
  • • Medical or Child Allowance
Groups or individuals that may be asked to release the above information (depending on program requirements include, but are not limited to:
  1. Previous landlords (including Public Housing Agencies)
  2. Social Security Administration
  3. State Unemployment Agencies
  4. Banks and other financial institutions
  5. Support and alimony providers
  6. Law enforcement agencies
CONDITIONS: I agree that a photocopy of this authorization may be used for the purposes stated above. The original of this authorization is on file and will stay in effect for one year and one month from the date signed. I understand that I have a right to review my file and correct any information that I can prove is incorrect. Dana Properties, LLC and Whitfield Mobile Home Park LLC are committed to compliance with all Federal, State, and Local Fair Housing Laws. It is our policy to offer our residences for rental to the public without regard to race, color, national origin, religion, sex, familial status, or handicap.
